1.1 PureLife Gloves, LLC, trading as PureLife Dental (“PureLife”, “we” or “us”) owns and operates the purelifedental.com website (the “Site”), which is hosted in the United States.
1.2 We are the controller of personal data obtained via our Site, meaning we are the organization legally responsible for deciding how and for what purposes it is used.
1.3 We take your privacy very seriously. This Privacy Policy explains to you, as a visitor to our Site ("you" or "your"), how and why we collect, store, use and share any personal information relating to your use of our Site. This Privacy Policy also explains your rights in relation to your personal information and how to contact us.
1.4 All employees and those with whom we share personal information must adhere to this Privacy Policy.

We collect personal information (such as name and email address) from you when you choose to receive marketing-related email updates upon registration on the Site for an online account. In some cases, we may sell or rent your personal information, including but not limited to our receipt of a rebate upon a product purchase. We will not share your personal information with third parties to directly market their products to you who are not part of our business operations or that are not within our supply chain of products and services.

4.2.1 Service Providers, Suppliers & Vendors
We may disclose or otherwise make information about you available to third parties that help with our business operations. Examples are shipping vendors, billing and refund vendors, payment card processors, professional advisors and companies that help us improve our product and service offerings on our Site. Your information may be stored and processed by our service providers or suppliers in the United States or other locations where the service providers or suppliers maintain facilities. We require our service providers and suppliers to keep your information secure. In addition, our service providers and suppliers may not use or disclose personal information for any purpose other than providing the services or supplies on our behalf.
4.2.2 Legal Requirements and Protection of PureLife and Others

You may opt-out of PureLife email marketing communications by clicking the "unsubscribe" link in the emails you receive or by sending an email to [email protected] with the subject line, "UNSUBSCRIBE". Please be aware that, even if you have opted-out of receiving marketing communications from us, we may still contact you for transactional purposes, such as customer service, product information, service or reminder notices, or recalls. We may also need to contact you via email with questions or information regarding your order, such as order status, shipment or refunds.
6. Other Websites
Our Site may contain links to other websites on the Internet that are owned and operated by third parties. This Privacy Policy does not cover the information practices of those websites linked to our Site. We encourage you to be aware when you leave our Site and to read the privacy policies and statements of each website that may collect your personal information.
7. Children and Minors
Our services are not directed to children. If a parent or guardian becomes aware that his or her child has provided us with personal information without their consent, please contact us. If we become aware that a child has registered for a service and has provided us with personal information, we will delete such information from our files.

8.1 We are committed to security, confidentiality and integrity of personal information in accordance with legal requirements. We take commercially reasonable precautions to keep personal information secure against unauthorized access and use and we periodically review our security measures. We are committed to processing your data in a secure manner and have put in place specific technical and organizational measures to prevent the personal information we hold from being accidentally or deliberately compromised.
8.2 We utilize current industry accepted encryption technologies and strengths to reduce the risk that others are able to view information passing between our web servers and your browser. Please be aware that these protection tools do not protect information that is not collected through our Site, such as information provided to us by e-mail.

8.4 We also conduct information risk assessments, we train our staff to understand the importance of protecting personal information, and we are responsibly managing access rights within the company. We include both physical security and IT security in our overall data security approach. We are diligent in selecting service providers that process personal information on our behalf so that they also ensure appropriate technical and organizational measures to protect the data.
8.5 We make reasonable efforts to notify individuals and regulatory authorities, as required by law, if we reasonably believe that personal information has been stolen, disclosed, altered or infringed by an unauthorized person.
9. Your Data Protection Rights
9.1 If you resident of a jurisdiction with data protection laws, we are committed to supporting your rights granted by such applicable data protection laws.
9.2 Under certain circumstances, you may have the privacy rights described in clause 7 below.

If we determine that the request warrants a fee, or that we may refuse it, we will give you notice explaining why we made that decision. You may have the right to ask us to make corrections to inaccurate or incomplete personal information about you.
9.7.3 Erasure
You may have the right to request the deletion of your personal information we have collected from you, subject to certain conditions and limitations under the law.
9.7.4 Restriction of Use
You may have the right to request the restriction of processing of personal information about you.
9.7.5 Opt-Out of the Sale of Personal Information
To the extent we sell your personal information as the term “sell” is defined under the California Consumer Privacy Act, you have the right to opt-out of the sale of your personal information by us to third parties at any time.

Compliance with the Sunshine Act
The Open Payments/Physician Payments Sunshine Act (“Sunshine Act”) was signed into law as part of the Affordable Care Act in March 2010
The Sunshine Act requires certain healthcare companies, including PureLife, to report to the federal government any payments or transfers of value made to “physicians” (such term defined to include dentists), with certain exceptions, such as discounts and rebates provided in the ordinary course of business. The Sunshine Act also requires companies to report any physician ownership or investment interests.
PureLife is committed to ensuring that the data we submit under the Sunshine Act is as accurate as possible and has robust processes in place to enable tracking and reporting in compliance with its obligations.
